Luminary Care Services can take a referral from Local Authorities, Social Workers and other partner agencies.
When a referral is received, our team will begin by assessing
As part of the assessment, we prefer to involve the young person in the process and invite them to view the accommodation – this gives them the opportunity to meet face-to-face with our staff as well as allowing us to get to each other. After carrying out the initial assessments, our staff will make a decision regarding our ability to accept the referral.
Where there are factors such as learning disabilities, behavioural issues or mental health exist, we work closely with other agencies to make sure professional help is available to our young people so these issues can be addressed and tackled in a way that would enable them to slowly take responsibility for their lives and build on their independence, thereby improving their life skills.
How to Refer
The process of referring is simple.
All you have to do to make a referral, is call our team on 07977 777 718, or send us an email to info@luminarycareservices.co.uk
When emailing, please include the following information
- Your name
- Your organisation
- A contact telephone number
- A brief summary of information about the young person
Emergency and Out of Hours Referrals
We also offer an emergency referral service to Local Authorities needing to place a young person urgently, at a facility that is supported by professional and caring staff.
To ensure the correct package is put in place, we will require a referral form. We do recommend that the young person joins us on either our emergency package or intensive package for up to 28 days before being offered additional support. This is to ensure the facility is the best option for the young person and allow them to settle in before being provided additional support.
